Application forms must be fully completed and signed, and presented with

1. Passport, valid for a minimum of six months
2. Two photographs.
3. Copy of confirmed return air ticket.
4. International Yellow Fever Vaccination Certificate, taken at least 10 days before entry into India.
5. Relevant supporting documents.
6. Foreign National residents must provide a copy of residence permit / Carnet de Extranjeria

If applying from Bolivia, completed applications, along with necessary documentation and fees can be deposited with

They will then send it to the Embassy. In such cases, US$ 28 courier charges each way as well as US$ 12 bank commission charges per set of applications must be paid extra.

Alternatively, the application can be sent directly to the Embassy by courier. Pre-paid return courier should be used or US$ 28 must be included in the fees as return courier charges. Fees should be paid by bank draft (add US$ 12 commission charges per cheque) or sent by transfer to the Embassy's account in Lima. Personal cheques are not accepted.
